Regional Announcements Membership Intake Forms: Posted: December 26, 2004The National office has re-designed, re-written, and re-named the "Prospective Member Acknowledgement of Anti-Hazing Policy and Hold Harmless" form and the "Member Acknowledgement of Anti-Hazing Policy and Hold Harmless" form. These forms were previous number PBS-2A and PBS-2B. They are now numbered PBS-5A and PBS-5B. They can be found on the national website and can also be downloaded on this website by clicking the Documents menu option and selecting PBS Forms option. If there are any questions or clarification needed in using these forms please contact your Collegiate Advisor, Area Intake Team Member/Leader, or State/Regional Director of Collegiate Affairs. |
